PLEASE NOTE: THIS QUESTIONNAIRE NEEDS TO BE FILLED OUT PRIOR TO YOUR MASSAGE APPOINTMENT. IF 24 HOURS PRIOR TO YOUR APPOINTMENT YOUR SYMPTOMS HAVE CHANGED OR BECOME WORSE, YOU WILL NEED TO CANCEL YOUR APPOINTMENT. IF YOU ARRIVE AT THE CLINIC WITH SYMPTOMS YOU WILL BE ASKED TO RESECHEDULE YOUR MASSAGE APPOINTMENT TO A LATER DATE. THIS IS TO ENSURE THE SAFETY OF OUR CLIENTS AND THERAPISTS.
If you have answered ‘Yes’ to questions 1, 3, or have checked off signs or symptoms, you may need to reschedule your appointment. If you have answered ‘Yes’ to both questions 4 and 5, you may proceed with your appointment.
